Veterans Memorial Brick Form

The bricks will be placed in the Memorial Pavilion Walkway in front of the Dudley Municipal Complex next to the monuments.

6-line bricks are $100.

Please contact the Veterans Services Office on the first floor with a check payable to the Town of Dudley. The Veterans Services Officer will need to see the following information: The discharge Document, the DD-214, and other associated information. If the DD-214 is not available, the Veterans Services Officer can submit a request for this document. 

**It is preferable to meet with the Veterans Services Officer to assemble proper documentation and correct military language as required.
