WANDERERS/LOST PERSON FORM -
A Wanderers Informaton Sheet is available to residents who have loved ones inflicted with Dementia or Alzheimers Disease. The form should be filled out in case the person wanders away or becomes lost. It provides the Police with critical information and allows the searching to start sooner with accurate information.
